Boswell Advocate - Community Health Worker

$22-25 per hour
Boswell is seeking a contractor (part time) Community Health Worker (CHW) to support Medicaid members with the difficulties they’re facing in life. CHW experience and knowledge of local social service resources is a must.
In these times of economic uncertainty, more people are struggling with their social needs – such as having enough money for food and rent. Boswell strives to help them access the healthcare and social service resources that they are eligible. Our goal is to address social needs before they impact people’s health.
As a Boswell Advocate, you will play a critical role in this mission. You will meet with individuals who are going to food pantries, understand what additional health and social concerns they have, connect them with relevant resources, and build a long-term relationship.
Responsibilities
Visit Boswell’s partner food pantries in person to meet and engage with patients
Conduct assessments, offer guidance, provide education, and make referrals
Help patients navigate to relevant health and social service resources (e.g. schedule medical appointment, enroll in SNAP), rather than just handing them a list
Engage in SMS-based and Telehealth communications with patients for follow-on conversations
Document visit notes
Continue to build knowledge of social services and relationships within the communities we serve to assist in expanding our partner network
Support patients from diverse cultures
What you bring to the table
Experience: 2+ years of experience assisting people with their social needs in a social service organization or healthcare setting
Credentials: CHW certification by the RI Certification Board, or completing the CHW certification within the next 18 months
Language: Proficient in English, Spanish speaker preferred
Expertise: Intimate knowledge of local, state, and federal benefits that help people address their social needs around food, housing, transportation, etc.
Typing skills: Comfortable with typing on a computer, as a significant portion of patient engagement will take place over chat
Strong communication skills: Ability to build trust and connection with patients through your compassion, empathy, and ability to inspire hope
